CURRENT ERAU STUDENTS TRYOUTS

Any student currently enrolled at Embry-Riddle who wishes to try out for any ERAU Athletics teams is required to fill out the ERAU Student Tryout Health History packet below. You are also required to provide valid documentation of a physical completed within six (6) months prior to your official visit. Your physical must be completed by an MD/DO. We will not accept a physical completed by a Chiropractor. If you have not had a physical within the last six (6) months, have your doctor complete the Tryout/Physical form below.

RECRUITS
