If you don't have it on your computer, you can download it for free here: https://get.adobe.com/reader/ You must save the document to your computer BEFORE you start filling it out. If you fill it out in the web browser and then try to save it, you will only get a blank form, and you'll have to do it again. Your parents will need to sign the insurance forms, if you are going to be using their insurance. They can sign it electronically as well, our you can print out your completed forms, sign them, scan them back to your computer, and upload them to the system.
