These forms are required for all high school and trasnfer students on an official recruiting visit, as well as current Embry-Riddle student tryouts.

RECRUITING QUESTIONNAIRES
All prospective student-athletes wishing to compete for Embry-Riddle are required to fill out a recruting form. Please select the sport of your choice below and complete the associated form. The recruiting form will be sent directly to the coach of that sport.
