PARENT CONSENT & WAIVER
The registration form, which includes a consent and waiver form, must be signed by a parent/guardian. All participants are also required to submit the Medical Release Form.

EQUIPMENT
Campers are required to wear basketball equipment.

REFUNDS
Tuition includes a $50 non-refundable processing fee. NO REFUNDS AFTER THE START OF CAMP.
